IMPORTANT HEALTH COVERAGE DOCUMENTS
Former Swagelok associates who were eligible for minimum essential health coverage provided by the Swagelok Health and Welfare Plan are entitled to a Form 1095-C for the prior tax year, which serves as proof of coverage offered. In accordance with federal legislation, Form 1095-C is only required to be issued upon request.
If you would like to request a free copy of your Form 1095-C for your records, you may do so by emailing askbenefits@swagelok.com or sending a request to Total Rewards 31500 Aurora Road, Solon, OH 44139. A paper copy will be mailed to the address on file within 30 days of receiving the request.
